This case involved sinus augmentation in the upper left maxilla with horizontal ridge augmentation in the premolar region using an autologous onlay bone graft (OBG) harvested from the mandibular ramus. Simultaneous implant placement was performed in the molar region, while the premolar site required prior augmentation, necessitating a staged approach with additional implant placement scheduled six months later. During the second procedure, a modified palatal pedicle roll-up connective tissue graft was performed via a palatal window to enhance buccal soft tissue volume. Six months later, implants were uncovered for bridge rehabilitation. This approach provides a stable foundation for successful implant-supported restoration in cases of severe posterior maxillary atrophy.
