About this video
In select cases, when mandibular anatomy allows, implants may be placed lateral or lingual to the mandibular canal, angulated to engage the cortical bone and align with the upper dentition. Bicortical fixation can be achieved; however, while lingual placement offers dual bicortical anchorage, it is a complex and high-risk procedure requiring advanced surgical expertise due to potential complications from penetrating the lingual cortex into the floor of the mouth. This procedure demands meticulous technique and must be digitally planned, as osteotomies are performed blindly. A surgical template is essential, with posterior fixation pins and anterior fixation (either pins or teeth), preferably using a fully guided surgical template.

Augmentation Procedure Prior to Implant Placement
Augmentation Procedure Prior to Implant Placement

A case of severe atrophy of partial edentulous posterior maxilla treated with sinus elevation procedure and horizontal and vertical autologous onlay block bone grafts.

Bilateral Inferior Alveolar Nerve Lateralization and Transposition
Bilateral Inferior Alveolar Nerve Lateralization and Transposition

Lateralization and transposition of the mandibular canal with simultaneous implant placement for fixed prosthetic rehabilitation, including immediate implant placement and provisionalization.

Ridge Preservation Following Removal of Infected Failed Implants, with Secondary Vertical Augmentation Using Autologous Onlay Bone Grafting (AOBG) in the Posterior Mandible
Ridge Preservation Following Removal of Infected Failed Implants, with Secondary Vertical Augmentation Using Autologous Onlay Bone Grafting (AOBG) in the Posterior Mandible

A comprehensive treatment approach for managing chronic infection and severe bone loss in the posterior mandible due to failed implants. The protocol includes immediate ridge preservation with xenograft material (GBR), followed by vertical augmentation using autologous onlay bone grafting, and a final stage of implant placement with connective tissue grafting.

AOBG Augmentation – "Greenstick Fracture Technique" - Horizontal and Vertical Bone Grafting

This presentation introduces a bone grafting technique using autologous bone blocks to achieve both horizontal and vertical augmentation.
