In cases of severe posterior mandibular atrophy, lateralization and transposition of the inferior alveolar nerve may be the only viable solution for achieving a stable fixed prosthesis. With careful surgical and prosthetic planning, advanced imaging, and precise execution, this technique enables successful implant placement in the posterior mandible. This case presentation showcases a patient with bilateral posterior mandibular atrophy, where the mandibular canal is positioned superiorly. A step-by-step demonstration of bilateral inferior alveolar nerve lateralization is provided, along with simultaneous implants placement on both sides. Immediate temporization was performed on the left implants, while a horizontal autologous onlay bone graft (AOBG) was placed on the right side. The presentation also includes follow-up after final prosthetic rehabilitation.
