In cases of severe posterior mandibular atrophy, lateralization and transposition of the inferior alveolar nerve may be the only viable approach to achieving a stable fixed prosthesis. With meticulous surgical and prosthetic planning, advanced imaging, and precise execution, this technique allows for successful implant placement in the posterior mandible. This case presentation highlights a patient with bilateral posterior mandibular atrophy, where the mandibular canal is positioned superiorly, along with untreatable anterior teeth. A step-by-step demonstration of inferior alveolar nerve lateralization on the right side and transposition on the left side is provided, along with anterior alveoloplasty. Guided simultaneous implantation and provisionalization are also presented.
