A bone deficiency in the right posterior edentulous mandible results in inadequate 3D bone volume for implant placement, necessitating augmentation. To address this, an onlay bone graft will be performed using a graft harvested from the adjacent mandibular ramus. Among the available surgical approaches for posterior mandibular atrophy, an onlay bone graft was chosen. Autologous bone harvested from the mandibular ramus enables simultaneous horizontal and vertical augmentation. The “greenstick fracture” technique was applied, involving precise osteotomy preparation using a Piezo device at the vertical augmentation site. This technique facilitates controlled fracturing, allowing a seamless transition between vertical and horizontal augmentation. By preserving bone block integrity and minimizing excessive manipulation, the graft was immediately stabilized, ensuring continuity and structural stability.
