This case involves the rehabilitation of the atrophic posterior mandible on the right side, where significant 3D bone atrophy is present in the region of the first and second missing molars. Due to the position of the mandibular canal, there is insufficient vertical bone height for implant placement. With only 5 to 6 millimeters of available bone, short implants were not a viable option, and ultra-short implants were not recommended. Among various surgical options to address posterior mandibular atrophy, an onlay bone graft was selected. Autologous bone was harvested from the mandibular ramus to achieve both horizontal and vertical augmentation. The “greenstick fracture” technique was utilized, involving precise osteotomy preparation with a Piezo device at the border requiring vertical augmentation. This allowed for controlled fracturing, ensuring a smooth transition between vertical and horizontal augmentation. By preserving the integrity of the bone blocks and minimizing excessive preparation, the graft was immediately fixed in place, maintaining continuity and structural stability.
