This case involved congenitally missing second premolars in the right maxilla and left mandible, along with a Class II crowded malocclusion in the maxilla. The treatment plan aimed to extract the impacted left maxillary second premolar and transplant it into the missing left mandibular premolar site. Preparation included extracting the deciduous tooth at the recipient site, followed by shaping the site using a digitally segmented pre-prepared demo tooth. The impacted maxillary second premolar was then carefully extracted with its dental sac intact and successfully auto transplanted.
